Job description
This position is responsible for implementing educational programs and maximizing community engagement activities. The Health Educator will work in collaboration with the Health Education and Services Coordinator to implement the community outreach, recruitment, and education plans for individuals and families with a strong emphasis on adolescent reproductive health outcomes (teen pregnancy and STD/HIV prevention). The position will engage and educate various sectors of the community (males, parents, youth, and agency partners).

Responsibilities:

  • Implement programs for youth and adults in home and community-based settings. Examples of topics include: communication skills, reproductive anatomy and physiology, STD prevention, birth control methods, life planning and goal setting.
  • Conduct home visits to recruit and enroll participants in the program
  • Encourage and support efforts to improve adolescent reproductive health outcomes.
  • Assist with ongoing community-based activities
  • Prepare information packets for sessions
  • Meet weekly with the Health Education and Services Coordinator to provide updates on activities in the community
  • Recruit, prepare, and assist the host for Home Health Parties
  • Work with partnering agencies and providers to act as a resource for neighborhood residents
  • Participate in other forums (school PTAs, health fairs, churchgatherings, etc.) to inform parents, adults, youth and service providers about Plain Talk
  • Organize and implement community outreach and engagement activities and events following the Community Outreach Plan
  • Participate in local, state, and national teen pregnancy prevention forums to stay informed of broader issues that may impact the Plain Talk program
  • Assist in developing annual program tasks and goals
  • Assist with other program-related tasks as requested by the program coordinator
  • Identify and attend community meetings where possible
  • Maintain appropriate records of activities, fidelity, and outcomes. Assist in updating and maintaining service data in project databases.
  • Report to Health Education and Services Coordinator
