About the organization: The Penn Center for Community Health Workers (PCCHW) is a national center of excellence for community health worker research, application, and dissemination. We developed IMPaCT, an evidence-based community health worker model, which has been proven in clinical trials and replicated nationally and has supported almost 25,000 historically underserved patients in Philadelphia. The results demonstrate improvement in chronic disease control, improved mental health, improved communication between patients and providers, and 34% fewer hospital days compared to matched controls.
CHWs are trusted members of local communities who promote health and well-being by supporting patients’ physical, mental, and emotional goals. They are hired and trained based on key characteristics, including innate qualities like good listening, empathy, and trustworthiness with the patients they support.
Key Duties and Responsibilities:
Organization leadership
- Oversee programmatic outcomes and continuous improvement for patient care across hospital, outpatient, and telephonic CHW work and ensure top CHW performance
- Manage PCCHW’s team training and CHW professional development
- Integrate CHWs into hospitals and primary care clinics and build strong relationships with clinical partners.
- Represent the Center in major health system initiatives related to population health and social determinants of health (SDOH).
CHW leadership
- Supervise up to 2 Program Managers who lead a team of CHWs
- Supervise and coach 3 CHWs to provide excellent care
- Analyze data on a regular basis and identify opportunities for improvement
- Develop and implement performance improvement strategies and plans
Minimum Requirements:
Required Education and Experience:
- A Bachelor’s Degree is required; a Master’s is
- 8+ years of experience in a management role with a track record of hiring and retaining superstars, and achieving program goals, including 5+ years of leadership experience in organizations focused on improving the lives of individuals who have experienced harm due to race and socioeconomic position
- Self-starter who can make strong connections within and outside of the Center while working in a hybrid in-person/remote office arrangement
- Comfort and enjoyment working with data to understand trends, troubleshoot, and innovate
