Health Federation of Philadelphia – Early Head Start Social Worker and Home-Based Services Child and Family Advocate

The Health Federation of Philadelphia is recruiting for two American Recovery and Reinvestment Act-funded positions.

The Early Head Start Social Worker provides case management, counseling and other supportive services for pregnant women and parents who have children enrolled in the Early Head Start program.  Responsibilities include assessment of ongoing psychosocial needs, home visits, connecting families to resources and assisting with family goal setting.  The social worker also participates in case conference for families, in-service training, individual supervision and other appropriate training and staff development activities.

Candidates should have a Bachelor’s degree in Social Work or related area with at least two years experience as a social worker or case manager with some background in Mental Health

The Child and Family Advocate conducts weekly home visits to pregnant women and parents with children birth to three years of age to provide information and support to families in the area of child development, health, nutrition and life skills. The position also implements screening and assessment  tools and develops monthly lesson plans for home based services.  Candidates should have an Associate degree in child development, early childhood education or related field min.  Experience working with parents of young children in home base setting is preferred.
