Organizational Overview:

The Trans-health Information Project (TIP) is a peer-led multi-service health education project serving trans and gender-variant people in the greater Philadelphia area. TIP provides group-level peer education and risk reduction as well as individual resource referral, risk-reduction counseling and case-level advocacy to reduce HIV transmission and other harms associated with trans-phobia, injection silicone and hormone use, disclosure of trans bodies or identity, and unprotected sex and sex work.

TIP is the only Trans specific space of its kind in Philadelphia, and offers a safe space to get information and free safer sex supplies, talk, get resources and referrals, use the TIP library and wi-fi, and get involved in and help shape trans-advocacy in Philadelphia.

TIP staff run a community Drop In Center, speak events and trainings, conduct outreach to build and strengthen relationships with external referral resources, conduct community based outreach and recruitment, conduct individual level counseling, coordinate referrals to additional resources, and run education groups, health awareness groups, support groups, and fun events where people can learn from and share with one-another.

TIP is seeking a full time program coordinator who will coordinate general operation of the Drop In Center, coordinate individual level counseling, prevention, and resource referral services, coordinate group level services, and coordinate adherence to service deliverables and data entry for all funded programs.

Duties and Responsibilities:

1.Conduct supervision with 3 staff and facilitate regular staff meetings.

2. Ensure that deliverables are met through internal referral of participants from existing TIP programs and active recruitment in the TIP Drop In Center and in the community.

3. Carry a limited caseload and/or conduct a group on a monthly basis.

4. Assist other staff with development of prevention plans and care coordination plans for participants.

5. Assist with screening during crisis situations that may require specialized psychiatric or medical referrals.

6. Speak at events and trainings, and sit on advisory groups and task forces.

Develop and strengthen referral relationships that provide TIP participants with access to external resources.

7. Conduct follow-up with service providers to determine outcome of referrals.

8. Coordinate accurate and comprehensive record keeping in charts, including information on client sessions and referrals, and ensure that other staff adequately record sessions and referrals.

9. Ensure that staff members perform data entry on a monthly basis using internal data bases and the Program Evaluation and Monitoring System (PEMS).

10. Provide services in accordance with standards set by TIP and the AIDS Activities Coordinating Office.

11. Work in a past faced environment and have general oversight over daily operations.

12. Participate in semi-monthly Coordinator meetings at Prevention Point Philadelphia.

13. Perform other duties as assigned by supervisor.

Skills Required:

Excellent oral communication skills – both one-on-one and for group presentations
Skill in communicating effectively with a variety of people of various socio-economic and educational backgrounds.
Ability to use a harm reduction approach.
Dynamic, creative, flexible, able to learn and adapt in a changing environment.
Ability to work independently and as part of a team.
Ability to meet the physical requirements of conducting street outreach for 3 hours per week.
Excellent written communication and organizational skills.
Basic computer skills (Windows/Microsoft Office).

Minimum Qualifications:

At least two years work experience with communities impacted by HIV/AIDS, substance use, poverty, and/or homelessness.
Direct service experience with one-on-one counseling and group facilitation experience preferred.
Knowledge of Philadelphia social service and medical systems.
Bilingual (Spanish) preferred.
Knowledge of HIV prevention; AACO HIV Counselor certification preferred.
Demonstrated commitment to advocacy and social justice, particularly as related to Trans-advocacy, harm reduction, HIV/AIDS, poverty, and homelessness.
