Philly Counts in partnership with Philly Community Alliance Against COVID-19 would like you to complete this 15-minute survey about opinions and perspectives on COVID-19, the vaccine, and public health in your community.
The online survey is completely anonymous and participants will receive a $15 Amazon gift card for completing it.
