Supports Coordinator Job Description:
Incumbent will provide direct services to consumers with various disabilities; and ensure consumers receive the services they require to live, learn, work and socialize in their communities.  This includes the ability to identify, provide and maintain regular supports to assure continuity and quality of care while maintaining compliance with necessary regulations.  Regional travel (own vehicle or public transportation) of approximately 50% is required.  Perform other duties as assigned.

Required Experience:

  • Experience must include coordinating several services at a time including providing socialization opportunities for individuals, the ability to teach individuals living skills as will as aid in therapeutic activities.
  • Requirements also include exceptional interpersonal, communication and organizational skills.  They must be able to work professionally and confidentially with people on all levels of the organization as will as with consumers, families and attendants.  Excellent computer skills are also needed.
  • Incumbent will be required to successfully complete resource management training within the first 90 days of hire.

Required Education:

Bachelor’s degree including or supplemented by at least 12 college-level credit hours in sociology, social welfare, psychology, gerontology or another behavioral science.
OR
A combination of proven experience and training which add up to 4 years of experience and education including at least 12 semester hours of college-level courses in sociology, social work, social welfare, psychology, gerontology or other social science.
