Job description
Summary:

Ensures case management compliance with Office of Supportive Housing (OSH) contract requirements. Serves as the case management liaison between Jane Addams Place and the OSH.

Ensures the provision of clinically appropriate, trauma-informed social work services to all families living at Jane Addams Place.

Primary Responsibilities:

  • Reflects in professional practice the mission and philosophy of Lutheran Settlement House/ Jane Addams Place.
  • Provides weekly clinical and administrative supervision of Family Advocates.
  • Supervises Family Advocates in the evaluation of resident services. Monitors resident progress toward established goals and objectives.
  • Supervises Data Savings and Fees Administrator to ensure resident compliance with OSH savings and fees requirements. Ensures timely collection of resident savings, maintenance of accurate records and completion of appropriate documentation related to savings and fess payments.
  • Acts as ETO administrator and ensures the successful collection, managing, and reporting of data in ETO software (Efforts to Outcomes is LSH’s data tracking software).
  • Meets with internal and external stakeholders to discuss proper usage of ETO.
  • Intervenes during periods of resident crisis to provide direction and support to Family Advocates.
  • Directs contracting and protocoling process to address resident noncompliance with Jane Addams Place and OSH expectations.
  • Maintains responsibility for promoting the Sanctuary trauma-informed philosophy of service provision within the program.
  • Facilitates weekly Client Progress Review (CPR) meetings with staff.
  • Ensures that housing packets are completed on a timely basis and are sent to OSH in a timely manner.
  • Manages caseload assignment.
  • Utilizes OSH Homeless Management Information System (HMIS) to monitor compliance with OSH case management standards.
  • Maintains collaborative relationships with community service providers including Department of Human Services (DHS) mental health and substance use providers
  • Develops and maintains contact with other agency staff to develop specialized services for mothers in residence.
  • Monitors provision of mental health services through FaSST provider.
  • Compiles data, completes information and submits OSH quarterly report.
  • Supervises MSW students in the completion of daily tasks, interactions with residents and implementation of case plans.

Skills :

  • Demonstrated clinical social work and administrative supervisory skills.
  • ETO experience preferred.
  • Comprehensive knowledge of trauma informed care.
  • Attention to detail.
  • Excellent organization skills.
  • Sound judgment.
  • Ability to communicate effectively orally and in writing.

Education/Experience:

  • Master’s degree in social work or related human services field. Prior supervisory experience is a plus.
  • Experience working with homeless individuals and/or families is helpful.
