Part Time Respite Case Manager – Women of Change As part of Project H.O.M.E.’s Winter Initiative, 5 Respite beds will be provided for women who are living on the street, from January through April. The Respite beds serve women who are chronically homeless and unable or unwilling to stay at traditional shelters due to mental health issues, substance use or compromised physical health. Services include on-site case management, medical care and mental health services.

Part Time Respite Case Manager – St. Columba

The St. Columba Men’s Respite provides 5 beds for men who are living on the street within a facility that already houses 40 men. It is open 7 days a week, January through April, 24 hours a day. The Respite serves men who are chronically homeless and unable or unwilling to stay at traditional shelters due to mental health issues, substance use or compromised physical health. Services include on-site case management, medication management, and meals.

The primary role of the Respite Case Manager is placement of Respite guests into more permanent housing. The Case Manger will build rapport and collaborate with guests to identify their needs and barriers to housing and develop a housing plan to address those needs. The Case Manager will help guests access services and benefits, work with other service providers and maintain progress notes in a database.

Candidates must have:

a BA in Social Work or related field;

3 years experience with mental health, homelessness, and substance use issues;

previous case management experience;

knowledge of Philadelphia resources;

proficient computer skills, excellent verbal and written communication skills;

20hrs per week
