Rising STARS Grant Application Posted

The Rising STARS grant application and instructions have been posted to the PA Keys website.  The Rising STARS initiative focuses on access to the highest quality services for hard to reach/hard to serve children and families by providing supports to providers as they move up the quality scale. Rising STARS grant eligibility criteria:

• Must be an active STAR 1 provider
• Have not already received a RISING STARS grant in the past
• Have not already received two STARS Support grants in the past
• Actively working on a Continuous Quality Improvement plan that includes moving up to a STAR 2 in a reasonable amount of time

Application packets are due by November 1st.  Application packets consists of: a completed FTE for the current Fiscal Year, Keystone STAR 2 Standard Status worksheet and the RISING STARS application completed in full.  Applications will be scored and all practitioners will be notified of their grant status around January 3, 2014.  Upon notification, successful applicants will be provided with the grant paperwork to complete.
