Aligning Forces for Quality: The Regional Market Project seeks to help communities dramatically improve the quality of the health care they provide for chronic diseases in ambulatory settings. The program will help up to six communities align three key drivers of quality improvement: performance measurement and public reporting; capacity to help physicians in the community improve the quality of ambulatory, chronic illness care; and consumer engagement.
