Description:
Youth Health Empowerment Project (Y-HEP), a program of Philadelphia FIGHT, offers a comprehensive range of health promotion and youth development programs using a harm reduction approach. Y-HEP is committed to supporting youth as they transition to healthy adulthood by providing the following programs: peer-based outreach, youth leadership and political education, and a non-judgmental drop-in center offering a family planning clinic, harm reduction counseling and groups, HIV prevention services, and therapy.

Job description:
The nurse practitioner is responsible for offering care in and providing administrative oversight of the family planning clinic. This includes:
• Maintaining Policy and Procedures
• Providing family planning services including screening for STIs, pregnancy, and HIV
• Assessing participants’ risk for acquiring or transmitting Sexually Transmitted Infections (STIs) (changed location in list)
• Providing acute care for youth aged 13-24
• General and sexual health counseling
• Counseling youth on reproductive health options
• Follow up on lab and pap smear results including linking clients with outside providers as needed for more involved follow-up care (e.g. colpos)
• Ordering medical and family planning supplies
• Attending monthly Family Planning Council Provider meetings
• Providing referral services for participants’ medical needs that affect their health and ability to change HIV-related risk-taking behavior
• Work with Y-HEP case managers and therapist to provide integrated patient care and support.
• Enrolling eligible women into Select Plan
• Maintaining lab results
• Performing administrative duties and chart reviews including QI; Overseeing superbill completion for all clinic services; Creating, updating and implementing new policies and procedures as necessary; managing other family planning clinic staff.
