Eliminating Disparities in Perinatal Health in West, South, and Southwest Philadelphia
Healthy Start is a federally funded program dedicated to reducing high rates of racial, ethnic and geographic disparities in perinatal health outcomes through outreach, case management, bereavement and education services.
The Division of Maternal, Child and Family Health of the Philadelphia Department of Public Health is soliciting proposals from qualified applicants to provide case management and health education services to pregnant and postpartum women in three selected geographic areas of Philadelphia – West Philadelphia (19104, 19131, 19151), South Philadelphia (19146, 19147, 19148) and Southwest Philadelphia (19139, 19142, 19143).
The application guidance will be available on or before March 18, 2005 at www.phila.gov/rfp/index.html or through the link on the Department of Public Health’s homepage (<a href="http://www.phila.gov/health/index.html">http://www.phila.gov/health/index.html</a>).
Applicants may attend an informational meeting on Friday, April 1, 2005, from 9:30 a.m. to 12:00 p.m. at 1101 Market Street, 11th Floor. The deadline for proposal submission is April 29, 2005, at 4:00 p.m.
