Project Manager, Penn Center for Community Health Workers

The mission of the Center is to improve health in high-risk populations through the effective use of Community Health Workers (CHWs). CHWs, hired from within local communities, provide social support, advocacy and navigation to low-income, high-risk patients. Our Center has developed IMPaCT, a CHW program which has been scientifically proven to improve patients’ access to care, engagement with healthcare, and hospital readmissions. We are a nationally-recognized CHW leader and have been featured in the New York Times, National Public Radio, and Forbes magazine. Our Center is part of the University of Pennsylvania Health System.

In response to our success within Penn Medicine and training requests from organizations across the country, the Center is growing. As a result, we are inviting applications from energetic and passionate individuals who want to supervise a team of Community Health Workers, help us continue to innovate and refine IMPaCT, and influence new models of healthcare for marginalized communities in Philadelphia and across the country.

Key Duties and Responsibilities

  • Lead a team of 5-7 CHWs. Your team will work with over 250 patients each year
  • Ensure continued quality of patient outcomes by producing regular reports and coaching your team to develop their skills
  • Integrate CHWs with care providers working in the hospital and in primary care clinics
  • Offer ideas for innovating and refining IMPaCT so we achieve the best results
  • Facilitate team meetings and support groups for patients
  • Conduct basic accounting for project expenses and program management

Preferred Qualifications

  • Leadership experience within community-based organizations, preferably in a supervisory role
  • Excellent understanding of healthcare and social service systems, including processes for enrolling in public benefits. Knowledge of resources in West and Southwest Philadelphia a plus
  • Experience with clinical social work and/or case management
  • Passion for improving the lives of low-income individuals and cultivating talented team members
  • Comfort producing reports and using data to drive performance results
  • Excellent oral and written communication skills
  • Ability to thrive in a dynamic organization, where flexibility, high performance, and creative thinking are strongly valued
  • MSW or MPH preferred
