Program Coordinator, Youth Outreach

One Day At A Time (ODAAT) Youth Services Division works with youth to enable them to grow as individuals and become a productive member of their community. This is achieved by providing customized support for their unique needs and building trusting relationships between youth leaders and adults. This position, funded by the Department of Behavior Health/Office of Addiction Services, is an exciting opportunity to work as part of an outreach team to serve the needs of youth at risk for drug use or self-identified use. This comprehensive youth engagement initiative for at risk youth will include prevention, early intervention and program development activities that are monitored through clinical assessment and evaluation.

Position Summary:
The Program Coordinator can expect to deliver supports and programs consistent with recovery planning and resiliency building. Youth life plans established with youth will target to enhance their capacity to function in all aspects of community living. Responsibilities include developing and implementing life skills programs based on youth life plans, providing case management to individuals on caseload, providing appropriate personal and physical care, crisis response, counselling and emotional support, where appropriate services may be extended to children in school or home with their families.

Key Duties & Responsibilities:

PRIMARY RESPONSIBILITIES:

  • Provide clinical support and case management under the Harm/Risk reduction and Trauma-informed service spectrum delivering ODAAT services to at risk youth and families
  • Provide brokerage/resourcing and advocate for youth and families using evidence-based practices. Provide general information and referral services to the youth for mental health, recreation, housing, employment, education, health and other services
  • Work collaboratively with youth, parents, youth service providers, schools, city staff and law enforcement to develop effective service delivery plans that ensure that youth have access to programs, services and opportunities needed for healthy development by:
  • Engaging youth to identify issues, needs, goals aspirations, skills, abilities and services needed as well as barriers to accessing these services
  • Providing youth with information, contacts and referrals needed to facilitate access to programs, services and opportunities
  • Engaging parents through existing parent groups, faith communities, etc., and provide education and information on issues related to parenting, etc.
  • Organizing and participating in public education seminars, workshops, groups and training for youth, parents and service providers
  • Initiate, develop and implement community outreach activities with youth and communities in Philadelphia. Facilitate connections between youth and their neighborhood and promote awareness of the positive role of youth in the community;
  • Develop an understanding of neighborhood demographics, and location of services, programs and community assets available in the local neighborhood;
  • Identify emerging community issues, service gaps and opportunities for action to build on community capacity.
