The Program Coordinator is a full-time position with the Training & Capacity Building (TCAP) department and reports to the Program Manager. The TCAP department serves as a community-clinical integration hub for delivering, scaling and sustaining evidence-based chronic disease self-management and prevention programs for at risk populations through its local, regional, statewide and national multi-sector partnerships. Currently these programs target diabetes, pre-diabetes, chronic disease, chronic pain and arthritis self-management among high risk disparate populations, such as African American and Latino/Hispanic populations and those living with disabilities and behavioral health conditions. Ultimately, TCAP’s work aims to simplify and increase access to evidence-based self-management programs.
The Program Coordinator is responsible for planning, implementing, and performing a wide range of activities and tasks to achieve the goals of an assigned portfolio of services and programs based upon grants and contracts of the department. This requires the coordinator to develop and maintain a thorough knowledge of the evidence-based programs within TCAP’s portfolio, as well as a comprehensive understanding of the partner network and target audiences within the assigned region(s), in order to expertly recruit and engage them in TCAP’s services and programming. Required is an ability to adapt oral, written, and nonverbal communication in a clear, professional and inclusive manner to a variety of audiences, including but not limited to, diverse community members; staff and stakeholders of community based organizations; health care and health plan executives and network providers; clinical and care management teams; health educators; and state and federal government agencies.
