Paid Community Health Meetings Across Philadelphia – Jul 23 – Aug 7

The Philadelphia Association of Community Development Corporations (PACDC) is working with the Healthcare Improvement Foundation alongside local non-profit hospitals on conducting a Regional Community Health Needs Assessment (CHNA). This process is used by local hospitals to evaluate physical, mental, and social determinants of health impacting our communities and the well-being of residents. The results of this analysis will inform the development of health system’s implementation plans to guide their work to meet these needs over the next 3 years.
This is an opportunity for community voices to be heard on pressing issues facing their neighborhoods!

Prospective participants must be residents in one of the zip codes of the meeting they are seeking to attend. If you have questions please email [email protected].
