Job Overview:
The Outreach Coordinator will provide: health education individually and in group settings (classes); proactive outreach; and health system navigation to the population of patients served by our primary care practices. The goal of outreach is to increase cancer screening rates among our patient population; improve disease monitoring improve chronic disease control and reduce risk for chronic disease. The successful candidate will participate in health fairs to promote Drexel, perform health screening and health education. This role will also assist with referral management for patients as needed to ensure they are able to successfully manage their health at home.
Qualifications:
1. Bachelors degree and C.H.E.S. certification preferred. High school diploma or equivalent required
2. Two or more years’ experience working in a healthcare setting, either hospital or medical office.
3. Demonstrated ability to use Excel at intermediate level.
4. Referral management experience a plus.
5. Knowledge of workflows to obtain records from outside organization.

Essential Functions:
1. The Outreach Coordinator will provide telephonic outreach to patients who:
a. Have not had a visit in our practice during the recommended time frame and work with the patient to schedule a convenient appointment.
b. Are due/overdue for preventive screenings and assist the patient with scheduling as needed.
c. Are due/overdue for disease monitoring (A1C, BP check etc.) and assist the patient with scheduling as needed.
2. Target patients for chronic care management activities based on selection criteria
3. Reviews disease registries and acts upon overdue and abnormal results.
4. Provides patients with lifestyle coaching throughout the year.
5. Coaches patients in self-management and works with patients to develop self-management health plans.
6. Prepares for and conducts in person health education classes (with up to 15 participants) .
7. Reconciles medications
8. Address barriers to managing their health and helps patients find solutions to the problems they encounter.
9. Arranges and manages referrals.
10. Develops and implements a mechanism for tracking referrals.
11. Analyze payer data and prioritize patients for outreach accordingly.
12. Track and analyzes data for quality improvement and regulatory purposes.
13. Meets with practices at least 6 times annually to review and prioritize patients.
14. Maintains regular communication with other panel managers, supervisors and clinicians involved in panel management.
15. Perform health screening at health fairs and other health promotion venues.
