Outpatient Case Manager, University of Pennsylvania

The Cohen Clinic at the University of Pennsylvania is seeking an LSW or LCSW to serve as the clinic’s Case Manager.  The Case Manager will work as part of a larger behavioral health care team within the Cohen Clinic at Penn to serve military veterans, service members, and family members. Candidates with interest or experience in serving a diverse veteran/military community are encouraged to apply. 

The Case Manager will work as part of a larger behavioral health care team within the Cohen Clinic at Penn to serve military veterans, service members, and family members. The Case Manager will provide risk assessment and safety planning support to incoming referrals as they are getting established in care. They will support incoming referrals who require a higher level, or different focus of care, in navigating access and connecting to outside resources. They will provide case management and care planning services for clients at the Cohen Clinic. Will assist clients and team members in identifying appropriate external resources for clients and assisting in connecting clients with resources. Will maintain an accurate internal directory of resources. The Case Manager will actively participate in the coordination of client care, including attending case consultation meetings, actively tracking internal referrals, and maintaining timely and accurate clinical documentation.  Will conduct follow up check ins after care to ensure continued client wellness. Must maintain strong working relationship with external partners, participating in some direct community outreach to support these working relationships.

The population treated at the Cohen Clinic is multicultural and diverse with respect to race, ethnicity, gender identity, sexual orientation, and socioeconomic status. Candidate would have the opportunity to serve underserved groups of veterans, including women veterans and LGBTQ+ veterans and family members. Accordingly, applicants with experience and/or interest in multicultural/diversity issues and working with diverse populations would be a good fit for the position.

Standard schedule is 8am-5pm M-F with one evening until 7pm. Cohen Clinic staff are currently eligible to work one day (pre-scheduled Monday or Friday) remotely. However, remote eligibility is pending clinic need. All client services are provided in the clinic for via telehealth.  Travel limited to local outreach events.
