Social Determinants of Health: NJ 2-1-1 Improves Health Outcomes

NJ 2-1-1 has produced a short video urging healthcare professionals to utilize their services to improve patients’ health outcomes. The video is based on research which confirms that factors like unstable housing, low income, food insecurity, lack of transportation and substandard education are just some of the conditions that have been proven to have a negative impact on overall health.
