Social Determinants of Health: NJ 2-1-1 Improves Health Outcomes
NJ 2-1-1 has produced a short video urging healthcare professionals to utilize their services to improve patients’ health outcomes. The video is based on research which confirms that factors like unstable housing, low income, food insecurity, lack of transportation and substandard education are just some of the conditions that have been proven to have a negative impact on overall health. Known as social determinants of health, these are the very needs that NJ 2-1-1 is dedicated to addressing. A simple call, text or chat (link is external) to 2-1-1 will open the door to a wide variety of community resources for life’s most basic needs.
NJ 2-1-1 reached more than 5,000 New Jersey family physicians and doctors of internal medicine with the following message:
