COMMUNITIES TAKING ON DIABETES
Community-wide efforts to prevent and treat chronic public health problems are gaining traction both here and abroad. In the U.S., there is particular interest in translating evidence-based medicine into quality improvement initiatives for conditions like diabetes. In a new Commonwealth Fund report, researchers from the National Rehabilitation Hospital Center for Health & Disability Research evaluated one such effort, Taking on Diabetes–a group of community health partnerships formed to improve diabetes care in Kansas City, MO, Albuquerque, NM, and Westchester County, NY.
