Congreso de Latinos Unidos, Inc. is currently seeking a Medical Case Manager for the Esfuerzo Program within the Health Promotion and Wellness Division at Congreso. The Medical Case Manager will oversee the delivery of mental health services and ensure compliance with all applicable local, State, and Federal MH guidelines for clients in Congreso’s Esfuerzo Program. This position will provide those living with HIV/AIDS a comprehensive continuum of services through critical linkages with community resources. In addition, the Medical Case Manager secures ongoing follow up with medical providers as well as intensive counseling aimed at treatment adherence. In addition, this position will also provide direct services while carrying a small clinical caseload.

Essential Functions
1. Maintains a caseload of people living with HIV/AIDS as required by contractual standards.
2. Conducts and completes comprehensive assessment for each client.
3. Assesses the client’s needs and completes an initial and quarterly Service Care Plan.
4. Completes and submits client referrals for support services: food/clothing, financial benefits, transportation, legal, medical, living situation, substance abuse, support systems, mental health,
cultural/language.
5. Provides ongoing client follow-up, risk reduction information and short-term emotional support. Medical Case Manager Page 2 of 4
6. Assists the Supervisor in establishing and maintaining affiliated agreements with other community based organizations that serve at-risk minority individuals, to increase access to case management services. This includes maintaining relationships with other medical providers.
7. Conducts regular face-to-face medical visits, home visits and hospital visits.
8. Conducts in-depth treatment adherence counseling for clients. Specifically but not limited to: Knowing what medication the client is taking; When the client is taking those medications; Possible side effects; Knowing and understanding the client’s CD4 cell count and Viral Load results.
9. Responsible for monitoring HIV lab results for each assigned client with the goal of securing stable health.
10. Responsible for basic translation and interpretation services for Spanish speaking clients.
11. Responsible for working directly with SSI, welfare, health insurance/medical coverage and related social services that are directly related to the client’s health.
12. In collaboration with the Administrative Team: identifies problems that may hinder access to services and takes action to resolve them, and advocates for solutions to these problems.
13. The Medical Case Manager will maintain clear, concise and complete records and documentation in a timely manner for each client as required by the funding agency.
14. Maintains daily, weekly and monthly service reports and submits these to supervisor for review as determined by him/her.
15. Ensures continuous communication with the client’s primary care physician, mental health provider, drug and alcohol counselor and any other provider that is servicing the client with the goal of ensuring a stable health.

Minimum Qualifications
2+ years of experience in counseling or social work with extensive work with substance abuse and/or HIV/AIDS issues and services
BA/BS degree from an accredited university or college with a concentration in Social Work, Counseling, Psychology, Nursing, or related field
Preferred Qualifications
Bilingual (Spanish/English)
