The Medical Case Management Coordinator is responsible for the daily management and quality assurance of assigned staff and services delivered to clients in Congreso’s Esfuerzo Program. The Medical Case Management Coordinator will oversee program development and administration to include coordination and oversight of the program’s direct client services including; counseling on treatment adherence, overall HIV medical care, home visiting, nutritional counseling, and linkages to supportive services. This position will ensure contract compliance and drive the attainment of programmatic goals.

The Medical Case Management Coordinator has internal contacts with the entire administrative staff and external contact with clients, community resources, and the community. This position has access to sensitive Congreso information and is expected to handle such information with integrity and professionalism. This position has regular contact with members of the community and is expected to represent Congreso in a professional manner.

The Medical Case Management Coordinator will report directly to the Interim Manager of Medical Case Management and Supportive Services. The Medical Case Management Coordinator will supervise assigned staff and will participate in department activities as necessary.

Essential Functions

Program Development and Administration

  • Coordinates and supervises all aspects of care services including the supervision of staff assigned to the direct services unit and assurance that appropriate services are provided within that unit; provide technical external or in-service training to new hires, including training in the clinical aspects of medical case management and proper documentation methods of client records; and evaluate the work performance of assigned staff through regular supervisory meetings.
  • Supervises assigned medical case managers, each retaining a caseload of approximately 50 cases.
    Ensures that staff collaborates with medical providers in order to ensure continuous medical care and treatment adherence.
  • Conducts case review sessions with staff of assigned service components in accordance with Agency and funding regulations, and documents same for review with the Manager and provides the Department Director with timely requisitions to meet programmatic needs of assigned unit.
  • Reviews service care plans and actual services provided to clients to determine quality of service delivery; document same for review with the Supervisor; and provides clinical support to program staff in determining appropriate service direction in any case, as well as terminations and/or discharges.
  • Reviews client records and CAREWare/UNIDAD data for quality assurance in order to maintain compliance with agency and funding source regulations, and notifies staff and Manager of Medical Case Management and Supportive Services when records are out of compliance.
  • Conducts supervision twice a month with each assigned medical case manager. Maintains a record of supervision notes.
  • Enters client and supervision notes and units into CAREWare.
  • Monitors and evaluates the team’s CAREWare Performance Measures on a quarterly basis, including barriers to treatment adherence, and Service Care Plans to determine specific needs and plans of action.
  • Responsible for intake referrals. Reviews referrals and assigns new cases to medical case managers. Updates Intake Tracker, Client List, CAREWare, assembles charts, and enrolls clients in UNIDAD. As per the intake protocol, monitors CAREWare and follows up with staff to ensure contact.
  • Schedules face to face office visits with clients to complete and submit applications for DEFA (Direct Emergency Financial Assistance) and HOPWA (Housing Opportunities for People With AIDS). Educates clients about the application processes. Ensures thorough accuracy of documentation, maintains records, and communicates as needed with PHMC.
  • Assists supervisor with distribution and reconciliation of PHMC Food Vouchers and SEPTA tokens.
  • Completes and submits monthly and quarterly reports, including narratives and internal stats, to the Program Manager as required by agency and funding sources.
  • Conducts PCM (Primary Client Model) Observations with each assigned staff (once every six months) to evaluate quality of service delivery and provide direction and support to staff.
  • Assures that case management services are provided in an integrated fashion by working with other community agencies, as well as, other Congreso programs.
  • Maintain accurate information on all aspects of HIV/AIDS including approved medical treatments and alternate therapies; and ensure program staff is up-to-date with all aspects of HIV/AIDS as well.
    Conduct other related projects as assigned by Director including, but not limited to: volunteer training, outreach initiatives, and programmatic support.

Contract Compliance

  • Reviews client records and forms of assigned service components to assure that they are in compliance with Congreso and funding source regulations, and notifies staff and the Director when records are out of compliance.
  • Ensure that all assigned service staff fully comply with the confidentiality of HIV-related information regulations.
  • Assists in the development, implementation and monitoring of all performance measures and revenue requirements of the program.
  • Prepare, submit, and maintain records of statistical and/or narrative reports to the Program Director as required by Agency, and funding sources.
  • Ensures that Counselors complete the required 20 hours of continuing education units by the funders timeline.

Management Duties

  • Provide management of department and staff through interdependent judgment and decision-making.
    Collaborates with Supervisor to regularly direct the work of, schedule, and provide staff development opportunities and training to department staff.
  • Perform management functions and/or make recommendations for the department such as hiring, firing, performance evaluations, disciplinary counseling, salary adjustments, workload distribution, communication, recognition activities, and employee relations.
  • Participate in the development of a departmental budget as part of the organization’s budget preparation process, and operate within budget limitations.
