Looking for model K-12 school/health professional partnerships

Community-Campus Partnerships for Health is looking for models of service-learning partnerships between K-12 schools and health professional schools (partnerships that involve K-12 students and/or health professional students in service-learning). If you are from either a K-12 school or a health professional school and you are part of such a partnership, please take a few minutes to tell us about it!
We’ve set up a simple online survey to make it easy to share some basic information. It will only take 5 or 10 minutes to fill out the survey. Please respond by Monday, February 13, if possible.

We’ll be compiling the information into a resource guide that will be available on our website. Survey respondents will have an opportunity to review and comment on a draft before it is finalized.
