Description:
ESSENTIAL RESULTS:
Up to date knowledge and application of TANF, PA Department of Public Welfare, and JEVS Policies, Procedures and Regulations
Attend in-services and external trainings;
Incorporate changes in daily operations as directed, based on regulatory changes;
Support agency/program policies, procedures and requirements
Attend agency program staff meetings as required
Performance Measures
Meet: Attend all required trainings
Process audits reveal changes have been incorporated 90% of the time within 30 days of the change
Exceed: Attend all required trainings and over 90% compliant
Strong partnerships with external and internal customers
Act as a referral agent, referring participants to external agencies and organizations for assistance with behavioral health issues.
Accompany participants to appointments as needed to assist participants in navigating the MH/D&A systems. Provide follow-up with referral agency
Communicate effectively and efficiently with case managers and the case management supervisor.
Build and sustain relationships with participants.
Identify appropriate resources in the community. Maintain a database of all resources.
Work collaboratively and interdependently with other team members
Meet: 85-89% positive customer satisfaction surveys
Exceed: 90% positive customer satisfaction surveys
Positive Participant Outcomes
Provide preliminary mental health and drug and alcohol assessments as needed.
Determine need for service and best course of action.
Participate in orientation and the My Self Assessment group activity.
Act as an advocate for the participant.
Develop an action plan and next steps for the participants
Follow-up and provide outcomes regarding participant status.
Document all contacts with participants; maintain accurate and timely records of all counseling sessions. Input all data in into CAPS and JEVS databases. Maintain the integrity and confidentiality of all participant information including electronic, hard copy and filing system.
Facilitate case conferencing sessions with staff and participants.
Provide immediate crisis intervention if needed
Guide participants in learning how to independently resolve issues and barriers.
Conduct small group sessions addressing “the signs and symptoms” of behavioral health issues such as anger management, depression, homelessness, alcohol & drug abuse, etc.
