Kynett Foundation issues RFP

To celebrate its 60th anniversary, the Edna G. Kynett Memorial Foundation has announced a special grant opportunity.

The Foundation is seeking proposals for interventions designed to address the burden of cardiovascular disease in vulnerable populations. Interventions must include an active community outreach strategy. The Foundation is particularly interested in outreach interventions that include community health workers, patient navigators, peer educators, counselors, or other outreach workers.

Projects should include partnerships between nonprofit community-based organizations and health care clinics, hospitals or other health care settings. A focus on prevention is a priority.

Proposals must address populations at highest risk for cardiovascular disease: those in underserved communities and/or racial or ethnic minority populations. They must also target low-income communities in the Delaware Valley: Philadelphia, Bucks, Delaware, Chester, Montgomery, Berks and Lehigh Counties in Pennsylvania; Camden County, New Jersey and New Castle County, Delaware.

Up to three grantees will receive awards of up to $60,000 a year for two years (a potential total of $120,000). The deadline to apply is October 15th.

Additional details are available here.
