GENERAL DESCRIPTION:

SEAMAAC welcomes applications for the position of Immigrant Family Wellness Case Manager. SEAMAAC is a nonprofit organization in Philadelphia, with the mission “To support and serve immigrants and refugees and other politically, socially and economically marginalized communities as they seek to advance the condition of their lives in the United States.”

POSITION SUMMARY:

The Immigrant Family Wellness Case Manager will support SEAMAAC’s family-based services for Asian American and Pacific Islanders (AAPIs) and other immigrant families. The Immigrant Family Wellness Case Manager will provide case management, data collection, and support to three programs:

  • Safe Families Program (domestic violence)
  • Community Resources
  • Health Access

This position is a hybrid role, with some in-person work and some remote work. In-person work will take
place at the Hansjörg Wyss Wellness Center, 1902 S. 8th St., Philadelphia.

Primary Duties/Responsibilities:

  • Provide case management to immigrant clients. Provide resources for basic needs, health access, safety planning, housing, public benefits, family support, employment, education, and other needs
  • Conduct case management intakes and initial screenings of new clients
  • Using electronic databases, complete timely and accurate data collection including writing case notes and other documentation
  • Provide emotional support to clients
  • Assist with planning and facilitating group activities for AAPI domestic violence survivors and other survivors of trauma
  • Actively participate in team meetings, interorganizational meetings, and staff trainings
  • Actively participate in organization-wide events, a minimum of 5 times per calendar year
  • Other duties may be assigned from time to time
