Description:
The Achieving Independence Center (AIC) is a “one-stop center” designed to help young people, ages 16-21, from the dependent (foster care) system achieve their future goals of self-sufficiency.

The AIC Housing Case Manager will coordinate the assessment, planning, and case management of all participants in the AIC Housing Component.

The AIC Housing Case Manager is responsible for the following:

1) Manage referrals of youth to the AIC Housing Component, including managing and prioritizing a waiting list.

2) Conduct eligibility and acceptability screening assessments of referred youth and complete client intake and ongoing paperwork consistent with program protocols, and in a timely manner.

3) Assist youth in locating, obtaining, and maintaining apartments. This includes being the primary liaison with identified landlords.

5) Assist youth in developing and implementing an individual plan for independence including housing, employment, education, life skills, and permanency goals.

6) Provide individual life skills counseling on an as needed and as directed basis.

7) Monitor youth attendance at school and employment.

8) Weekly visits at each participants’ apartment.

9) Participate in on-call rotation with other staff.

The ideal candidate will also possess:

A commitment to positive youth development and a strengths-based approach.

Familiarity with the Philadelphia metropolitan area.

Training in (or willingness to learn) adventure-based counseling techniques.

Additional Qualifications:
Bachelor’s Degree in Education, Human Services, Social Work or other related field.

Experience working with youth regarding goal planning and/or homelessness issues.

Willingness to travel throughout the city on home visits.

Child Abuse, Criminal Background and FBI clearances.

Available early evenings and weekends.

Enjoys working with teens.
