Position Description

Position Title:  HIV/AIDS Medical Case Manager

General Description:  To provide clients who have HIV/AIDS with links to services that will improve their compliance with medical regimens and treatment adherence.  Reports to Medical Case Management Supervisor.

Typical Duties Will Include, but are not limited to:
1.    Providing intensive case management and supportive services to persons living with HIV/AIDS throughout Montgomery County

2.    Assist clients in accessing direct supportive services through building relationships in the communities with doctors and other social service agencies

3.    Assess client program eligibility quarterly as needed by funder

4.    Working in collaboration with the Housing Coordinator to ensure safe housing and services associated with permanent housing

5.      Working with Mental Health Facilitator to recruit their clients for the Healthy Relationships intervention

6.    Participation in continuing education program through the Case Management Coordination Project

7.    Counseling clients on risk reduction interventions through HIV prevention education

8.    Preparing monthly statistical summaries for each funding source in categories outlined by funders

9.    Maintaining accurate client files for continuous quality management reviews by Case Management Supervisor and Funders as needed

10.    Other duties as assigned

Education and Experience:  Bachelor’s degree in Social Work or related Human Services discipline required.  Experience in providing HIV/AIDS services is preferred.  Fluency in Spanish a plus.  Must have car and current car insurance.
