Maternity Care Coalition (MCC), a dynamic $8M nonprofit, works to improve maternal and child health and well-being through the collaborative efforts of individuals, families, providers and communities.
Healthy Start is a National Program, provided by MCC in conjunction with the Philadelphia Department of Public Health, aimed at addressing the issues of infant mortality, low birth weight and racial disparities in perinatal outcomes.
MCC seeks a full-time Case Manager responsible for identifying and providing culturally sensitive in-home services to pregnant and newly parenting women including: assessment, referral, follow-up, monitoring and emotional support. The Case Manager screens women for depression; helps clients access and utilize all necessary health and human services; helps women maintain their families’ primary health care; facilitates community health education programs; attends health fairs; maintains careful records of all client contacts both in a paper file and in an online database. This position is based at our West Philadelphia site location.
Bachelor’s of Social Work (BSW), counseling, public health or closely related field required. We seek an individual with excellent organizational and people skills. Must have strong time management and problem solving skills; excellent written and oral communication ability; and knowledge of community resources. Excellent computer skills, valid driver’s license and clean driving record required.
