Healthy Start Case Manager/Family Advocate

Maternity Care Coalition (MCC), a dynamic $7M nonprofit, works to improve maternal and child health and well-being through the collaborative efforts of individuals, families, providers and communities.

Healthy Start is a National Program, provided by MCC in conjunction with the Philadelphia Department of Public Health, aimed at addressing the issues of infant mortality, low birth weight and racial disparities in perinatal outcomes.

*** THIS IS A TEMPORARY POSITION: May 2-July 22, 2011***

MCC seeks a Case Manager responsible for identifying and providing culturally sensitive in-home services to very high-risk pregnant and newly parenting women including: assessment, referral, follow-up, monitoring and emotional support.  The Case Manager provides home-based services including: screening women for depression, helping clients access and utilize all necessary health and human services; helping women maintain their families’ primary health care; facilitating community health education programs; attending health fairs; maintaining careful records of all client contacts both in a paper file and in an online database.

Master of Social Work degree is required. Candidates without an MSW will NOT be considered.  We seek an individual with excellent organizational and people skills. Must have strong time management and problem solving skills; excellent written and oral communication ability; and knowledge of community resources.  Must be computer literate.  Valid driver’s license and clean driving record required.
