Healthy Communities Access Program Demonstration Project Funds – Deadline: August 20
Examples of support include: providing research-based decision support to improve quality of care and assist with translation of research into primary care community-based practice, measuring the efficacy of skill building and training to assess those with most significance for self-management, cultural competency training and the provision of patient/family centered care.
