Health Zones Initiatives, CVS Health Foundation – May 1

CVS Health and its Foundation are pleased to announce an open request for proposals (RFP) for our Health Zones initiatives, where we support cross-sector collaborations working to improve health outcomes for those living with chronic conditions in Philadelphia, PA. This funding opportunity is for a group of registered 501c3 organizations working together on integrating healthcare and social care for people living with chronic conditions in the Philadelphia community. This will be a three-year grant for up to $2.25 million in total.
