Job Summary: Responsible for providing reproductive and sexual health education related to disease prevention and healthy behaviors, provide limited health screenings (i.e. HIV/STD testing, interviews for risk reduction planning, etc.) reproductive and sexual health care system referrals and resources identification for persons seeking additional reproductive and sexual health screenings, diagnosis and treatment within the AccessMatters network. This position will provide testing and screening services to diverse populations with a focus on men who have sex with men (MSM). Outreach and HIV testing will be conducted during day, evening, and late night hours at various targeted locations throughout the city. Settings for outreach and testing may include bars and clubs, shelters, drug treatment facilities, and neighboring communities via mobile unit. The Health Resource Specialist II is responsible for ensuring that all clients who require linkage to medical care and other services are linked in an efficient and timely manner. The Health Resource Specialist II is responsible for overseeing and scheduling all Health Resource Specialist I staff and outreach volunteers.
This position will require late night (10pm-3am) evening hours and weekends. This position reports to the Senior Manager, LGBT Health Initiatives.
Essential Functions:
- Promote the program in the communities at risk and identified in planning, in community clinics, organizations and institutions who are stakeholders in the improvement of reproductive and sexual health, through presentation and lectures.
- Assist in coordination of care and support services for individuals and families to improve health outcomes, based on care provider referral plan and client assessment, identifying additional needs.
- Assist with development and coordination of all testing activities, and collaborate with other community-based organizations and community stakeholders to create events and activities for MSM communities.
- Provide sexual and reproductive health education to improve client health literacy, increase active participation in care decisions, and to maintain health care access.
- Increase quality health outcomes; establish goal-setting that includes risk reduction, adherence counseling and disclosure services where appropriate.
- Collect process data, maintain and regularly update program database.
- Meet regularly with Senior Manager to report and review progress.
- Maintain communication with Senior Manager on a weekly basis.
- Maintain communication with clinic staff at identified sites to access target population to identify status or/ and coordinate services.
- Maintain communication with relevant healthcare and support services entities that may provide diagnostic, treatment and additional social support for clients.
- Serve as Team Leader for the Health Resource Specialist l and/or outreach volunteers.
Knowledge, Skills, and Abilities:
- Proficiency in the provision of risk reduction counseling sessions in sexual and reproductive health.
- Proficiency in the administration of HIV testing technologies in clinical and community settings.
- Proficiency in the administration of STD testing technologies in clinical and community settings.
- Knowledge of Microsoft Office suite, websites, social media, Internet and data entry advantageous.
- Ability to work without on-site supervision.
- Ability to facilitate educational programs in diverse settings
- Ability to engage consumers in diverse settings (clinical, community settings).
- Ability to communicate effectively verbally and in writing with individuals or groups.
- Ability to perform with a high level of confidentiality, discretion and integrity.
