Health Educator/Coach
Essential Function
The Supportive Older Women’s Network (SOWN) is eager to hire an experienced, enthusiastic professional to provide health and wellness workshops to an older, primarily female, population living in West Philadelphia.
The Educator/Coach supports the Growing Healthy Lives Together Project, funded by the Administration on Aging, designed to create a comprehensive healthy living program that includes consumer education, emotional support and motivation and access to resources. The goal of the project is the strengthening of physical and emotional well-being of older adults living independently in senior apartment buildings. The position is ideal for an experienced health educator who enjoys working with an aging population and who is skilled in collaborating and building relationships with community based organizations.
Primary Duties
Utilizing a team approach, design, coordinate and facilitate Healthy Living workshops in senior, subsidized apartment buildings in West Philadelphia
Assess the strengths and needs of the older adult and provide one on one coaching to help promote healthy living, problem solving and action planning
Assist older adults in forging links to needed services
Train an aging population to access and utilize web based health related resources
Build strong relationships with service coordinators/social workers/building managers in all senior apartments served by the project
Gather and maintain appropriate data in order to evaluate program effectiveness
Assist with reporting to funders and other interested community partners
Education and Experience
The successful candidate possesses a Master’s degree in Health Education, Nursing, Nutrition or related field and at least 3 years of experience working with elders and/or providing health education in a community setting. The individual must be a flexible team player, comfortable working within multiple systems, with superior computer, communication, organizational and relationship building skills. The most crucial aspect to success in this challenging and exciting position is a genuine desire to work and interact with an aging population striving to maintain their independence in the community while struggling with chronic health problems and other vulnerabilities.

Growing Healthy Lives Together provides on-site, easily accessible, convenient and participant- driven health and wellness programs to older adults living independently in subsidized, senior housing apartments. Currently based in West Philadelphia, this project is replicable throughout the city. The advertised position travels among the various sites and necessitates driving a car.
