Horizon Foundation for New Jersey Accepting Applications for Health Center Initiative Phase 2

The Horizon Foundation for New Jersey has launched Phase 2 of its Health Center Initiative to support 501(c)(3) nonprofit Federally Qualified Health Centers, free/volunteer clinics, and independent clinics in New Jersey that expand access to healthcare for the state’s uninsured and underserved residents.
The $2 million, two-year grant initiative will provide funding to applicants whose proposed initiatives facilitate individuals’ access to primary and specialty care that helps prevent and manage health concerns and promotes overall well-being.
The Phase 2 Request for Proposals has been structured to complement the federal healthcare reform law. For example, special consideration will be given to health center applicants that seek funding to implement the law’s Patient-Centered Medical Home model. Special consideration will also be accorded to proposals that incorporate a nurse-managed health center model.
HCI 2 grants will provide funding of up to $125,000 annually for up to two years ($250,000 maximum). Grant projects of any duration, from one month to two years, will be considered. There will be one funding round only, so applicants should request funding for the full term of their project.
