Family Empowerment Services Case Manager, Congreso

Congreso de Latinos Unidos, Inc. is currently seeking a full-time Family Empowerment Services (FES) Case Manager to work within the Family and Housing Services Division. The FES Case Manager will handle those cases referred by schools, in-house Congreso referrals or walk-ins by an individual or family seeking assistance. Services are geared toward providing social and structural supports that will strengthen families and prevent entry or re-entry into the child welfare system. The FES Case Manger will work directly under the supervision of the FES Coordinator or FES Program Manager.

Essential Responsibilities
 Provide in home services to at-risk families or individuals no less than twice a month or as needed
 Provide Case Management services (crisis intervention, advocacy, family assessment and planning)
 Develop service plans for clients and families
 Ability to professionally safeguard sensitive and confidential information/data of clients
 Maintain up-to-date client files and input client records into DHS and agency databases
 Regularly attend and participate in staff meetings, supervision, staff trainings and case reviews
 Ability to work outside of normal business hours to accommodate clients when necessary
 24 hours on-call availability to respond to and support family emergencies

Minimum Qualifications
 2-3 years of case management experience providing supportive services to community members
 Bachelor’s degree in Social Work or related social science field

Preferred Qualifications
 Bilingual (English / Spanish)
 Experience working with TANF recipients
 Familiarity with the North/Eastern North Philadelphia community and surrounding resources
