Position Description
The mission of the Center is to improve health in high-risk populations through the effective use of Community Health Workers (CHWs). CHWs, hired from within local communities, provide social support, advocacy and navigation to low-income, high-risk patients. Our Center has developed IMPaCT, a CHW program which has been scientifically proven to improve patients’ access to care, engagement with healthcare, and hospital readmissions. We are a nationally-recognized CHW leader and have been featured in the New York Times, National Public Radio, and Forbes magazine. Our Center is part of the University of Pennsylvania Health System.
In response to our success, the Center is growing. As a result, we are looking for a passionate, energetic and versatile leader to direct IMPaCT at Penn Medicine. This individual will play a key role in leading the program during an exciting period of growth. Most importantly, s/he will have the opportunity to influence new models of healthcare for vulnerable communities in Philadelphia and across the country.
In addition to our work at Penn Medicine, our Center partners with other healthcare organizations to create local CHW programs. In response to the demand for these services, we are also accepting applications for directors to help build new programs across the country in sites such as Boston and New York City. Below is a summary of key responsibilities and preferred qualifications for IMPaCT directors
Key Duties And Responsibilities
- Lead a talented and diverse team of more than 35 employees, with 6-8 direct reports
- Ensure continued quality of outcomes for more than 2,000 patients each year
- Recruit, select, train, supervise, and evaluate staff in alignment with Center goals
- Oversee day-to-day programmatic, financial and personnel operations
- Provide vision and strategic leadership for IMPaCT’s growth
- Work with Center leadership to innovate IMPaCT with input from CHWs, clinicians and patients
