Job description
The Director of Integrated Health Services is a Family Medicine physician who is responsible for optimizing the programmatic excellence of the services provided by Project HOME’s Federally Qualified Health Center (FQHC). The Director leads a growing interdisciplinary team of clinicians at a multi-site FQHC, including medical, behavioral health, dental and mobile health services. The team includes clinicians who are directly employed or contracted to provide services on-site and through written referral agreements. The Director provides the leadership to drive meaningful change and integration of services, employing best practices in measurement and performance improvement. The Director should be comfortable utilizing data to drive improvement. The Director engages regularly with the senior and executive leadership team to design and refine strategic plan goals, then creates a clear link between high level strategic goals and daily work of all clinical staff. This process is facilitated by engaging in and promoting regular leadership “rounding” in a way that fosters a culture of open communication and continuous improvement. KEY AREAS OF RESPONSIBILITY:

  • Provide supervision and direction, either directly or through a designee, to all FQHC medical, behavioral health and dental clinicians. Direct reports include Director of Clinical Services, Director of Behavioral Health Services, and Director of Dental Services.
  • Provide direct patient care as a Family Medicine physician; participate in an on-call, after hours rotating staff schedule; register with the State Board of Medicine as a supervising or collaborating physician for mid-level providers (physician assistants and nurse practitioners).
  • In collaboration with Vice President of Healthcare Services and Director of Operations, ensure staff at all levels have adequate tools to obtain and measure progress toward goals, utilizing QI methodology, visual management tools, and process mapping.
  • Conduct regular “rounds” at medical, behavioral health, dental, and mobile programs and sites to observe, mentor and coach these front line teams in a manner that creates a culture of accountability and fosters continuous improvement.
  • Serve on Project HOME’s Senior Management Team; attend Executive Management Team meetings as needed.
  • Conduct provider recruitment and retention activities, including the writing or editing of position descriptions.
  • Perform semi-annual and annual performance evaluations of direct reports; review performance evaluation results of all within her or his chain of command.
  • Provide significant input in development of the annual operating budget for clinical services.
  • Participate in weekly scheduled leadership team meetings with Vice President, Healthcare Services and the Director of Operations to review financials (productivity, patient services revenue, other revenue, and expenses).
  • Regularly review financial results with direct reports in a team setting to facilitate collaborative understanding and problem-solving of financial issues.
  • Recommend and execute plans to improve the FQHC’s financial performance as needed.
  • Participate in the Continuous Quality Improvement (CQI) Planning Committee.
  • Spearhead application for NCQA Certification as a Patient-Centered Medical Home.
  • Become familiar with regulations, licensures, credentials and contractual stipulations governing FQHC operations.
  • Assist the Vice President of Healthcare Services in addressing any grievances by patients, employees, contracted personnel, or third party payers to the extent such grievances and complaints are related to the services provided by FQHC clinicians
  • Collaborates closely with Senior Leadership in street outreach, residential, education and employment departments to ensure the FQHC continuum of care supports and integrates these services in a way that is seamless to the client and wisely uses resources.
  • Attend Board of Trustees, Healthcare Committee and/or Consumer Advisory Board meetings as needed.
