Philadelphia FIGHT is an AIDS Service Organization that recently became a Federally Qualified Health Center (FQHC). Our goal is to end the AIDS epidemic within the lifetime of people living with HIV now. This position is in FIGHT’s Education Department, which offers a wide range of educational, psychosocial, and risk-reduction programming for youth and adults living with HIV and those at high risk.
The Coordinator of Navigation Services is responsible for direct oversight of the navigation services of the Counseling, Testing, Referral and Navigation Department, and facilitating the coordination of all navigation services throughout Philadelphia FIGHT. This position works closely with the Counseling and Testing team, the Youth Health Empowerment Project, the AIDS Library New Chapters program, The C a Difference Hep C team, The PrEP team, the Institute for Community Justice, Ryan White Case Managers, and the FIGHT Community Health Centers medical providers and front desk staff to ensure that high risk HIV-negative clients are enrolled in navigation services and that HIV-positive clients are linked to medical care and services. This position supervises a team that provides short-term case management and crisis intervention services to clients who identify primarily as gay men of color and who are enrolled in the navigation program. The Coordinator of Navigation Services represents Philadelphia FIGHT both on site and in the community, in a courteous, efficient manner that reflects the high level of integrity and customer service mandated by Philadelphia FIGHT. This will include the supervision of some late-night testing events.
Hours
Full-Time, 40 hours per week; exempt
Tasks and Responsibilities
- Coordinates a navigation system across Philadelphia FIGHT that includes Y-HEP, the AIDS Library, the PrEP program, the health centers, and the counseling and testing department
- Holds monthly all-FIGHT navigation meetings with people from each of the relevant programs.
- Creates processes, procedures, and reports to ensure best practices in client care across FIGHT and prevent duplication of services.
- Spearheads the provision of a social services database to track all social service interactions with clients.
- Oversees the AACO navigation grant targeting men who have sex with men (MSM) of color.
- Oversees all components of FIGHT’s CDC grant that include navigation services and linkage to care.
- Demonstrates competency in all facets of client-centered care including intake, assessment, risk reduction planning and referrals;
- Ensures that high risk HIV-negative clients are linked to appropriate services through the Prevention Navigators
- Ensures that HIV-positive clients are linked to HIV care and additional risk reduction interventions (ARTAS, CLEAR, TEACH) as outlined by grant contracts.
- Ensures that Navigation staff provide risk reduction interventions in accordance with the standards established by the Centers of Disease Control;
- Educates and builds relationships with other providers of services
- Trains navigation staff in local resources for client referrals, prevention counseling, harm reduction, motivational interviewing, and the ongoing research about high-risk populations, HIV prevention strategies, and other topics as they arise.
- Trains staff on housing, recovery resources, mental health resources, Hep C, and other possible client needs.
- Convenes and directs case conferences;
- Provides weekly supervision and clinical support of Navigation staff as well as staff yearly performance evaluations.
- Maintains individual client charts for all relevant programs (Navigation, ARTAS, CLEAR).
- Tracks client referrals as outlined by grant guidelines and tracks successful linkages to FIGHT Health Centers
- Submits monthly and quarterly reports related to deliverables.
- Works in tandem with the C a Difference Hep C navigators and the PrEP retention coordinators
- Works collaboratively with project funder, Philadelphia Department of Public Health, AIDS Activities Coordinating Office, to meet grant obligations, reporting requirements, and timelines;
- Remains informed on national PrEP and PEP guidelines and local resources
