Community Primary Care Challenge Grant (CCG) RFA #07-07-12 funds grantees to increase access to health care in Health Professional Shortage and Medically Underserved Areas. This RFA includes two funding opportunities. Tier 1 funds new or expanded primary care medical or dental programs with a maximum award of $200,000. Tier 2 funds the establishment of Community Health Centers with a maximum award of $500,000. For both Tiers, the matching funds requirement is 25% of awarded funding. Applicants may apply for one Tier of funding only.
Eligible applicants may apply for:
Tier 1 Funding:
New or expanded community primary care medical or dental programs
$200,000 maximum award
25% of awarded funds must be matched
Tier 2 Funding:
The establishment of new Community Health Centers in service areas where none exist
$500,000 maximum award
25% of awarded funds must be matched
Potential applicants:
Not-for-profit community-based organizations and health centers
Local county/municipal government entities
Community health improvement partnerships
Former Community Primary Care Challenge Grantees are welcome to submit applications. Current grantees with grants ending on or before 9/30/2008 may apply.
