Children’s Hospital of Philadelphia (CHOP) is seeking a Community Implementation Specialist to join our interdisciplinary team focused on community-clinical partnerships. The Community Implementation Specialist will work in partnership with the Division of Social Work to help advance family-centered care and the mission of Healthier Together, a 5-year initiative to improve the health of children in West Philadelphia.
The Community Implementation Specialist will fill a critical role in helping build community partnership and engagement around a community resource map and database. This resource mapping initiative seeks to create more effective and efficient ways of addressing families’ social determinants of health that impact children’s health and wellness. Additionally, the role will coordinate and provide strategic support to a Medical Legal Partnership across two sites at CHOP.
This position is ideal for a social worker or community health professional who is a highly motivated team player with demonstrated experience with interdisciplinary collaboration. This position requires a high level of organization with the ability to problem-solve independently, build fast relationships with clinical and community teams, and exhibit professionalism in multiple interdisciplinary settings.
