Community Health Worker – Adult Care and Transition Team, CHOP

This position will support the Adult Care and Transition Team (ACTT). The Community Health Worker (CHW) will be interacting with young adults and caregivers that are transitioning from CHOP to adult medical care. The young adults served by ACTT have complex, chronic diseases and/or developmental disabilities. The CHW will collaborate with social workers, RN care managers, and care coordinators in support of these patients.
The CHW will act as a community liaison to support patients and families in better managing their medical and social needs until goals are reached as defined by the health care team. This includes making home visits, connecting families to community resources, and communicating family needs with the larger care team. The CHW will work in both clinical and community-based settings, including patients’ homes and community agencies.
