Community-based services for older adults

REQUEST FOR LETTERS OF INTENT
United Way is looking for organizations currently providing community-based services for older adults.
United Way of Greater Philadelphia and Southern New Jersey has partnered with the Pennsylvania Department of Aging to implement and evaluate innovative approaches to the delivery of supports called Community Networks of Care (CNC). CNCs will assist older adults to remain in their homes and communities for as long as possible and will provide supports to approximately 300-500 seniors. The CNC model will pilot in up to 3 targeted neighborhoods over the next 3 years. If successful, it is hoped to provide systemic change for sustainable and replicable community supports for older adults in Pennsylvania.
Coordinating Agencies will launch CNCs and sustain operations for the duration of three years. Up to three Coordinating Agencies will be selected for up to 3 neighborhoods. Coordinating Agencies will conduct outreach that connects older adults to community resources, available services and volunteer opportunities.
United Way will host two question and answer sessions for potential Respondents on November 29th from 3-5 PM and December 4th from 2-4 PM at 1709 Benjamin Franklin Parkway in Philadelphia.
