Maternity Care Coalition (MCC), a dynamic nonprofit, works to improve maternal and child health and well being through the collaborative efforts of individuals, families, providers and communities.
Healthy Families America (HFA) is a nationally recognized evidence-based home visiting model developed by Prevent Child Abuse America in 1992 and designed specifically to work with families who are at-risk for negative childhood experiences including child abuse and neglect.
MCC seeks three (3) self-motivated individuals to fill Case Manager positions within the HFA MOMobile program. The Case Managers will provide culturally sensitive group education and in-home case management to pregnant and newly parenting women, including assessment, referral, follow-up, monitoring and needed supportive services.
The successful candidates will have strong organizational and time management skills, excellent written and oral communication, and an interest in maternal and child health. Bachelor’s degree in social work, counseling, public health or closely related field preferred. Valid driver’s license with clean driving record required.
Locations:
Two positions are located in North Philadelphia (one bilingual English/Spanish required)
One position is located in Upper Darby serving families in Montgomery County’s Lansdale, Souderton and Hatfield townships (bilingual English/Arabic preferred)
