Description of Position:
The Case Manager is responsible for client assessments, service plan development, monitoring of client progress and needs, relapse prevention and accompanying clients to appointments as needed. The Case Manager is also responsible for making referrals to community resource agencies and monitoring service delivery, staff consultation, maintaining proper files and documentation, collaborating with multiple entities for client service coordination, and facilitating monthly groups (e.g.: drug and alcohol, life skills etc), Performs related duties as assigned.
MINIMUM EDUCATION , EXPERIENCE, OTHER REQUIREMENTS:
Must have a Bachelor’s Degree in Social Work or related field. Must have prior work experience with Drug and Alcohol counseling and/or Mental Health. Individual must be well organized, detail oriented, computer literate, willing to work in a trauma informed environment and be able to relate to and work with multi-diagnosed individuals. Candidate must have an appreciation for, and understanding of, the Mission of The Salvation Army.
