CASE MANAGER
The Case Manager will be primarily responsible for counseling, referrals and service coordination. Case managers will build on the Bridge’s extensive knowledge of neighborhood health, social service, behavioral health, educational, faith based, and recreational/social socials services to coordinate and follow-up on specific goals established in the Academic Support Action Plan. Case managers will also conduct group life skills classes and support activities during the on-site program hours. Case managers will have at least a bachelor’s degree, and will be trained in the unique issues of at-risk youth and their families. The Case Manager reports to the BIPS Program Director. The Case Manager also carries administrative responsibilities to ensure that internal systems are well coordinated and are aligned with external sources and payers.
CLINICAL RESPONSIBILITIES:
• Works as a team with the Therapist/Counselor in providing systemic treatment to adolescents and their families both on-site and in their homes using various professional integrative and collaborative modalities.
• Assist with intake and assessment information using the Bridge bio-psycho-social assessment form and a recovery-oriented, culturally competent assessment approach.
• Provide weekly family-focused case management/case coordination to approximately 16 adolescents and their families (including home-based family case management), depending on each family’s individual needs.
• Participate in Service Plan meetings to develop discharge criteria and community reintegration goals with the adolescent and their family.
• Develop strong relationships with community-based organizations and resources throughout the city to help client and her family establish linkages to appropriate community resources.
• Ensure that discharge planning is included in treatment planning and is the focus of case management from the time of admission.
• Conduct psycho educational, Life Skills, and other didactic groups for the residents and family members.
• Collaborate with representatives from other systems involved in the youth and family’s treatment process.
• Participate in all required case consultation meetings, staff meetings and trainings.
• Help develop ASAP plans with the counselor, guidance counselor and youth.
• Maintain accurate up to date records.
• Other duties or activities as assigned by the Program Director.
ADMINISTRATIVE RESPONSIBILITIES:
• Share in the responsibility of tracking due dates for all Service Plans, collecting necessary reports, submitting materials to Program Director for quality review, and ensuring that information is delivered to DHS in a timely fashion.
• Accept other administrative duties to support the viability and growth of the program along with adherence to policies and governing bodies.
SKILLS:
• Knowledge, understanding and experience working with at-risk youth, substance abuse, trauma and/or mental illness
• Strong knowledge of community resources relevant to this population.
• Knowledge and experience and ability to provide family partnership/parent training
• Sensitivity to racial, cultural, and economic and willingness to adopt interventions that work for different groups.
• Understanding of a strengths-based, recovery-oriented approach and a willingness to adopt this approach.
• Knowledge of high-risk youth, family systems, mental health disorders and substance abuse.
EXPERIENCE:
At least 2 years related experience
EDUCATION REQUIREMENT:
Master’s Degree in social work, psychology, family therapy or related field preferred.
Bachelor’s Degree in similar field with case management experience acceptable.
Valid driver’s license and vehicle required.
