Provide case manager services for 200+ court ordered Family Court clients in need of behavioral

RESPONSIBILITIES:
• Provide case management services for 200+ chemically dependent Family Court clients
• Case management services include: monitor clients’ activity in various treatment programs, provide a monthly census of active and inactive treatment clients, client data entry of admission and discharges, and obtain client discharge summaries from the treatment provider
• Write weekly progress reports to the various courts
• Duties would consist of preparing, emailing, and faxing the reports to various parties such as Judges, Attorneys, DHS worker, City Solicitors, and Child Advocates
• Monitor appropriateness of client’s level of care and advocate for change if needed

SKILLS:
• Must have knowledge of behavioral health services and some familiarity with Philadelphia area social service agencies
• Knowledge of CPCMS
• Must be able to work independently and be self-motivated
• Must have excellent verbal and written communication skills
• Must have excellent organizational skills

EXPERIENCE:
• Behavioral health treatment and social services
• Must have experience with substance abuse assessment, minimum of 1-2 years drug and alcohol knowledge and/or experience working in Family Court
• Knowledge of co-occurring disorders strongly preferred

EDUCATION REQUIREMENT:
Bachelors Degree with one to two years related job experience

SALARY:
Pay Grade 14
