Case Manager, Dignity Housing

Dignity Housing is seeking a Case Manager, who will be responsible for providing supportive services including home-based visitations and office consultations at least once a month. The Case Manager is required to conduct basic needs assessments and work with residents to develop a service plan delineating goals and action steps toward achieving housing stability and self-sufficiency.

The Case Manager is also responsible for monitoring progress, updating the service plan, and providing referrals and resources related to basic family needs, employment, education, housing and health and wellness. The Case Manager is required to keep detailed records and documentation of residents’ progress and status—the position requires excellent writing skills, and ability to handle volume of writing and reporting. The Case Manager is also required to work in conjunction with other Dignity Housing staff and collaborative partners. Additionally, the Case Manager is responsible for working with a caseload of 18-34 residents.

This position is full-time, 35 hours per week. Salary is $17-18 per hour, depending on experience. Some evening and Saturday work is required.

Qualifications:

Bachelor’s Degree required; 2 years experience preferred. Familiar with standard case management procedures, documentation and reporting; strong knowledge of government benefits and community resources within a social service environment; must have strong background or experience with substance addiction; strong knowledge of domestic violence and mental health pathologies; proficiency in most general computer operation systems and applications required, including: Word, Excel, Outlook, and Client Database. Excellent time management and organizational skills; excellent written and oral communication skills; highly energetic and creative; able to work in a team; passionate about improving the lives of others while maintaining appropriate boundaries.

Requirements:

• Valid Driver’s License and vehicle with insurance
• Current criminal background and child abuse clearances
• Resume and one to two-page maximum writing sample
