Case Manager/Community Engagement Specialist, SOWN – Nov 1
Posted by SOWN on October 25, 2022
SOWN seeks a Case Manager/Community Engagement Specialist (CM/CES) to provide services for SOWN’s GrandFamily Resource Center (GFRC), a one stop-shop for grandparents raising grandchildren who are in need of support and resources. GFRC programs take a wholistic, trauma-informed approach to offer grandparent-headed families mental health support, educational tools, and community resources.
The CM/CES will work closely with the GFRC Coordinator to support clients with referrals to community services and resources, and provide other relevant information to support the families’ needs. S/he will participate in community outreach and recruitment, and serve as a point of contact to connect grandparents with other programs. S/he will have demonstrated experience working with parenting adults and/or older adults in an urban setting using a trauma-informed, culturally and linguistically sensitive approach. S/he will hold at minimum a bachelor’s degree, preferably in social work or related field and have a minimum of two years’ experience in social services. While no vehicle is required for this position, it will require the ability to travel throughout Philadelphia.
This position is scheduled during SOWN’s standard operating hours, Monday – Friday, 9 AM – 5 PM, with occasional evenings and weekends. SOWN provides a hybrid work model and an employee benefits package.
